If you are attending our clinic for the first time, you will need to complete our New Patient Registration Form. This can be done online or alternatively, by downloading a PDF version. Online forms are automatically submitted to the rooms. PDF forms will need to be brought with you on the day of your consultation, faxed to (02) 9389 0122 or emailed prior to your appointment. You will need the latest free Adobe Reader to save a filled in version of the PDF form.

What to Bring to Your Appointment
- Your referral
- Your current glasses or contacts prescription
- A list of any medications that you are taking
- Your Medicare Card
- New Patient Registration Form or fill in on the day
Dilating eye drops may be needed to perform part of your examination.
These can take some time to work, and may also affect your capacity to return to work or to drive a car safely for a few hours after the appointment.
